Healthy Youth, Healthy Families Program

The Somerset County Health Department provides a FREE at-home health, nutrition and physical activity education program to both children and parents/guardians. This family oriented lifestyle change program will equip the whole family with education, tools and resources for sustainability of their lifestyle changes. The program includes a free food scale, a measuring cup and spoon, healthy eating magnets, handouts and more, a free healthy meal, and a physical activity gift at the end of the program. Free produce will also be given in the summer and fall.

Topics discussed include understanding food labels, understanding nutrition, eating on a budget, healthy cooking, steps to success and physical activity. This program is provided by our health educators at no cost to participants, and will engage youth up to 18 years of age in activities designed to enhance healthy eating behaviors and increase physical activities through educational face-to-face contact. Home visitations include four one hour sessions. A language hotline is provided, if needed, to eliminate language barriers.

Speak to your physician today about referring you to this program. They will complete a form and fax it to us at 410-651-4083 or they can call 443-523-1719. The referral form can be found below or at your health care provider’s office.
