The Thrive By Three program at the Somerset County Health Department provides care management, pregnancy and postpartum education, baby supplies, infant and toddler safety education, and referrals to maternal & child resources.
We offer baby supplies including diapers, baby wipes, pack ‘n plays, strollers, car seats, and baby gates.
Who is eligible?
Services are provided to pregnant & postpartum women and children from infancy to 3 years old regardless of insurance status.
integrated maternal health services (IMHS)
IMHS provides nurse case management of pregnant and postpartum women.
Based upon need, the nurse may make referrals to obstetrician (OB) care, primary care, mental health care, substance abuse care, dental care, USDA’s Special Supplemental Nutrition Program for Women, Infants, and Children (WIC), Judy Center, and Healthy Families Lower Shore.
All patients are educated about the importance of prenatal care, signs of pre-term labor, infant safe sleep, urgent maternal warning signs, and other topics as needed.

Pregnant and postpartum women living in Somerset County identified as high risk.
